STATEMENT OF SYMPTOMS FORM

THERE ARE NO TRAVEL RESTRICTIONS FOR DOMESTIC TRAVEL IN THE UNITED STATES. (revised 3/21)

SYMPTOMS STATEMENT:

By signing this statement, I attest that no one in my party is currently exhibiting any symptoms of COVID-19 such as:
1. Have been in close contact with a confirmed case of COVID-19 (NOTE: Healthcare workers caring for COVID-19 patients while wearing
appropriate personal protective equipment should answer “no” to this question.)
2. Fever in the last 48 hours
3. Respiratory symptoms such as sore throat, cough, or shortness of breath
4. Changes in a person’s sense of taste or smell

Since March 2020 Rustic has had new processes in place, including but not limited to heightening safety awareness, closing our office, changes to cabin
cleaning and disinfecting procedures, and implementing a self check-in/out procedure. Basically this is our existing late check-in procedure applied to
both phases.

A Welcome Bag with the cabin key, booklet, and kindling will be on our Office porch with each guest's name. Just check the name on the bag, grab and go.
Firewood, charcoal, pet fees, and extra person fees are to be paid with CASH or via VENMO. Any room balance can also be paid in this manner or on the
credit card on file. Though our office will be closed we will be on hand to address your needs. We can certainly open it if needed. Just ask
one of our staff members, call or leave a note.

CABIN HANDBOOK & JACUZZI TUB OPERATING INSTRUCTIONS REVIEW

I have received the Cabin Handbook and - where applicable -, the Jacuzzi Operating instructions and agree to leave the cabin in the condition it was
found upon check-in and to operate the Jacuzzi tub per it’s instructions. I understand that my credit/debit card will be charged if there are any damages
to the cabin, tub, or if excess cleaning is required.

Payment Policy

Deposit of 40 percent is required upon making reservation

Balance due on arrival

Cancellation Policy

Cancellations require a minimum or 14 days' notice before arrival date for refund of deposit less a $35.00 cancellation fee.

If you cancel less than 14 days before arrival and we can re-rent the cabin for the dates reserved, we will refund the deposit paid less the cancellation fee of $35.00

No shows will be charged full amount.
